Submit a completed Application 2. Pay application fee of $22.50 3. Purchase a minimum of 1 case of Ez-trim 105+ product. ($276.00 per case) 4. Shipping & Handing is 4% of the total (minimum $5.00) and liquids are an additional .75 per Qt. If you do not have a tax number, tax will be charged on the retail cost. You will then keep the tax you collect when you sell the product. If you have a tax number (it does not cost to get a tax number) no tax will be charged on your order. Please supply a copy of your tax certificate. You can do this by calling the above phone number or write to the above address. The application and order can be taken over the phone and the forms will be mailed to you, with your first order. We accept all major Credit Cards.
